Hair Loss Treatment for Men: What Works at Each Stage
Why you can trust this guide: myhair.health earns a commission on some laser devices and nothing on finasteride, minoxidil or transplants. See our affiliate disclosure.

The right hair loss treatment for men depends on how much hair is left: finasteride and minoxidil protect hair you still have, so they work best early, while only a transplant puts hair back where the scalp has gone smooth. The American Academy of Dermatology says the men who see the best results start soon after they notice loss. Waiting costs hair.
Which Norwood stage are you?
Your Norwood stage decides which hair loss treatment makes sense, so find it before you buy anything. Norwood published the scale in the Southern Medical Journal in 1975: stage 1 is a full hairline, stage 7 is a horseshoe band around the sides and back. Stages 2 and 3 are mostly temples. Stage 3 vertex adds a thin crown. By 4 and 5, front and crown are both retreating and the bridge between them narrows. Sudden or patchy loss is something else, and the minoxidil label says to see a doctor first.
Hair loss treatment for men, stage by stage
Hair loss treatment for men splits cleanly by stage: drugs carry the early years, drugs plus extras defend the middle, and surgery or a clipper owns the end.
| Norwood stage | What you see | Main options | Realistic goal |
|---|---|---|---|
| 1 to 2 | Temples just starting to move | Photos now; finasteride once loss is confirmed | Keep what you have |
| 3 and 3 vertex | Clear M-shaped hairline, maybe a thin crown | Finasteride, plus minoxidil on the crown | Hold the line, regrow some crown |
| 4 to 5 | Front and crown thinning, bridge narrowing | Finasteride plus minoxidil; laser cap as an add-on; transplant consult | Protect the bridge and crown |
| 6 to 7 | Bald top, horseshoe left | Transplant if donor hair allows; scalp micropigmentation; a clean shave | Coverage, not regrowth |
Norwood 1 to 3: the cheapest years to act
Finasteride earns its reputation early, because it slows the hormone-driven shrinking of follicles that are still there. In two trials of 1,553 men aged 18 to 41, Kaufman and colleagues (Journal of the American Academy of Dermatology, 1998) counted 107 more crown hairs than placebo after one year and 138 after two, while the placebo group kept losing. One honest catch: the Propecia label says efficacy in receding temples has not been established.
Topical minoxidil is the over-the-counter partner, and the men’s 5% label limits it to the top of the scalp, “vertex only”. In a 48-week trial of 393 men, Olsen and colleagues (JAAD, 2002) found 5% produced 45% more regrowth than 2%. Store-brand 5% carries the same active ingredient, as our Kirkland minoxidil review shows, and does minoxidil work walks through the numbers.

Norwood 4 to 5: combine, and protect the bridge
In the middle stages the goal shifts from regrowth to defense, and treatments get stacked. The AAD notes finasteride can be taken alongside minoxidil, and a dermatologist may add a prescription. Why the pair tends to beat either drug alone is the subject of finasteride vs minoxidil. In a 2022 JAMA Dermatology network meta-analysis, Gupta and colleagues found oral minoxidil at 5 mg a day gave the biggest gain in terminal hairs at 24 weeks, while dutasteride, finasteride’s stronger cousin, led on total hair count. Both are off-label for hair in the US and need a doctor, which is why we wrote up how doctors choose oral or topical minoxidil.

Laser caps fit here as an add-on, never a replacement. Lanzafame and colleagues (Lasers in Surgery and Medicine, 2013) tested a 655 nm helmet on 44 men at Norwood 2a to 5, and after 16 weeks hair counts rose 39 percentage points more than with a look-alike sham. We earn on some caps, so read that study for what it is: small and short. Prices sit in our laser cap ranking.
Norwood 6 to 7: moving hair, not growing it
A hair transplant is the only treatment that puts hair back on a bald top, because it moves follicles from the back and sides. Donor hair is the hard limit. The AAD adds two points: men in their 20s may be asked to start medicine and wait, and hair loss can continue after a transplant. Not every man wants surgery, either. A shaved head is a legitimate plan, and the only one with no side effects.
What the side effects look like
Finasteride’s side effects are real but uncommon in the trials. In year one, the Propecia label reports decreased libido in 1.8% of men versus 1.3% on placebo, and erectile dysfunction in 1.3% versus 0.7%. The same label lists post-marketing reports of depression, suicidal thoughts and sexual problems that continued after stopping, so any mood change goes straight to your doctor. Minoxidil’s usual trouble is early shedding and itch; see minoxidil side effects.
Where to start this month
Start by naming your stage, photographing hairline and crown under the same light, and booking a doctor’s visit about finasteride. Then be patient. The Propecia label says three months or more before benefit, the AAD says minoxidil can take six to 12 months, and stopping gives the gains back. Judge it at a year. Quick answers
What are the big 3 hair loss treatments?
Online, “the big 3” means finasteride, minoxidil and ketoconazole shampoo. Only the first two are FDA-approved for male pattern hair loss. Ketoconazole’s best-known hair study (Piérard-Franchimont, Dermatology, 1998) was small, and its authors called for larger trials. Our guide to the best shampoo for thinning hair covers that study and the later ketoconazole and zinc pyrithione trials.
Can men’s hair grow back?
Thinning hair often can. At two years in the finasteride trials, investigators rated 80% of men on the drug as having more hair, against 47% on placebo, per the Propecia label. Scalp that has been smooth for years usually needs a transplant.
What vitamin is lacking if my hair is falling out?
No vitamin is approved to treat male pattern hair loss, which runs on genes and hormones. If shedding is sudden or diffuse, ask your doctor for blood work before buying a supplement.
Is there a new hair loss treatment for men coming?
Clascoterone 5% solution, which Cosmo Pharmaceuticals plans to sell as Breezula, beat its vehicle in two phase 3 trials with 1,465 men, announced in December 2025. It is not FDA-approved for hair loss yet. One to watch, not to buy.
Male hair loss is mostly a timing problem. The man who starts at stage 3 simply has more hair to defend than the one who waits for stage 5.
